What is Participant-Directed HPC?
Participant-Directed HPC allows individuals or their authorized representatives to direct how care is provided while receiving ongoing in-home support.
Do you provide respite services?
Yes. We offer both Community Respite and Residential Respite services.
What is Supported Living?
Supported Living helps individuals live as independently as possible in their own homes with appropriate support.

Yes. Adult Day Support includes supervised activities, community engagement, and skill development.
Yes. We offer DD Waiver Nursing and Waiver Nursing Delegation (WND) as authorized.
Yes. All services must be authorized through an Ohio DODD waiver.
Referrals may come from SSA teams, case managers, families, or authorized representatives.
No. Intake submissions help us understand needs but do not guarantee service availability.
We currently serve Franklin County and Central Ohio and are actively working toward expanding into neighboring counties.
No. In emergencies, please contact 911 or local emergency services.
Yes. Family involvement is encouraged, especially for Participant-Directed services.
